Following up on previous posts, please use this survey to define your situation - if you would like to be part of our Rehabilitation BETA Program, please add your contact details.
This is an invitation to add your experience of working with water fed poles and any injury, regardless of whether the cause can be attributed to using the water fed pole. We are looking for candidates to partake in our Rehabilitation Program, or you can simply have your information presented statistically in our White Paper to OSHA, USA (you can remain anonymous).
The Survey consists of questions as follows:
About Your Situation : (but not your ID - you can elect to remain anonymous)
History and Current Health - Summary of Your Activity
PAIN : Identification of Muscle, Ligament, or Tendon Injuries
REDUCED RANGE OF MOTION : Identification of Joint Injuries
NUMBNESS AND WEAKNESS : Identification of Nerve Injuries
We are seeking details ranging from basic identification of injury, to in depth descriptions of your injuries (optional) in the following areas:
FINGERS, HANDS, WRIST, ELBOW, SHOULDER, NECK, and BACK.
